成果の概要

本研究の目的は、高齢の切除不能進行大腸癌患者に対する国際的標準治療の確立である。高齢者大腸癌患者を対象に、フルオロピリミジン＋ベバシツマブ療法に対する、試験治療のフルオロピリミジン＋オキサリプラチン＋ベバシツマブ療法の無増悪生存期間における優越性をランダム化比較第III相試験にて検証する。副次的にQOL評価および高齢者機能評価を並行して行う。高齢者機能評価による大腸癌化学療法の個別化をめざす。今年度は最終解析を行った。実施症例数は251例であり、登録患者の年齢中央値は79歳（70-91歳）、PS 0が134人、PS 1が99人、PS 2が18人、男性135人、女性116人、転移臓器個数は1臓器が106人、2臓器以上が145人であった。75歳以上のPS 0-1が93%と比較的fitな患者が多く登録されていた。本試験のdecision ruleは、有効性のprimary endpointとしてB群がA群に対してPFSで優越性を示すとともに、OSのHRの点推定値が0.8以下となることとした。primary endpointであるPFSのHRは0.837（90.5% CI: 0.673-1.042）、片側ｐ値=0.086とB群のA群に対する優越性を示すことはできなかった。また、OSのHRの点推定値は1.058と0.8を下回ることはできなかった。また、有害事象は予想通りB群で多い傾向にあった。一方、本試験ではQOL評価としてEQ-5Dを評価したが、B群のA群に対するOdds比は0.94であり、B群がA群に対して優れていることを示すこともできなかった。以上より、切除不能進行大腸癌の高齢患者における初回治療として、標準治療であるフルオロピリミジン＋ベバシズマブ療法に対して、フルオロピリミジン＋ベバシズマブにオキサリプラチンを併用することによる優越性を検証するためのランダム化比較第III相試験を行った。オキサリプラチンを併用することによりPFSの延長を示すことはできずOSの延長も認められなかった。さらにオキサリプラチンを併用することで有害事象の頻度および重症度が増すことになった。以上から、切除不能進行大腸癌の高齢患者の初回治療としてフルオロピリミジン＋ベバシズマブ療法が推奨される。
